전북특별자치도가 병원 치료를 마친 환자의 돌봄 공백을 줄이고, 지역사회 중심의 통합돌봄 체계 구축에 나섰다.

전북자치도는 지난 17일 도청에서 도내 의료기관과 시군이 참여한 가운데 ‘퇴원환자 통합돌봄 연계서비스 업무협약’을 체결했다고 18일 밝혔다.

이번 협약은 퇴원 환자가 지역사회에서 안정적으로 생활할 수 있도록 의료·돌봄·복지 서비스를 연계하는 협력체계를 마련하기 위해 추진됐다.

협약에는 전북자치도와 도내 시군, 전북대병원, 원광대병원, 예수병원, 정읍아산병원, 군산의료원, 남원의료원, 전주대자인병원, 전주병원, 전주고려병원, 동군산병원, 정읍한국병원, 고창병원 등 12개 병원이 참여했다.

협약에 따라 의료기관은 퇴원을 앞둔 환자의 건강 상태와 돌봄 필요도를 살펴 지원 대상을 조기에 발굴하고, 지역사회 돌봄이 필요한 환자를 지자체에 연계하게 된다.

지자체는 연계된 환자에게 방문진료와 재가 돌봄, 생활지원 등 맞춤형 통합돌봄 서비스를 제공해 퇴원 이후에도 지속적인 관리와 지원이 이어지도록 할 계획이다.

전북자치도와 협약기관들은 앞으로 대상자 발굴과 서비스 연계를 강화하고, 지역사회 통합돌봄 체계가 안정적으로 운영될 수 있도록 협력해 나가기로 했다.

방상윤 전북자치도 복지여성보건국장은 “전북은 고령화 속도가 빠르고 의료 취약지역이 많은 만큼 병원 치료 이후 지역사회 돌봄 연계가 매우 중요하다”며 “이번 협약을 계기로 병원에서 집으로 이어지는 의료 안전망을 구축해 퇴원환자가 지역에서 안정적으로 생활할 수 있도록 지원하겠다”고 말했다.

Jeonbuk Special Self-Governing Province is taking steps to reduce the care gap for patients who have completed hospital treatment and to establish an integrated care system centered on the local community.

Jeonbuk Province announced on the 18th that it signed a 'Discharge Patient Integrated Care Linkage Service Agreement' at the provincial office on the 17th, with the participation of medical institutions and local governments in the province.

This agreement was pursued to establish a collaborative system linking medical, care, and welfare services to ensure that discharged patients can live stably in the local community.

A total of 12 hospitals, including Jeonbuk University Hospital, Wonkwang University Hospital, Yesu Hospital, Jeongeup Asan Hospital, Gunsan Medical Center, Namwon Medical Center, Jeonju Daejin Hospital, Jeonju Hospital, Jeonju Goryeo Hospital, Dong Gunsan Hospital, Jeongeup Hankook Hospital, and Gochang Hospital, participated in the agreement.

According to the agreement, medical institutions will assess the health status and care needs of patients approaching discharge, identify support targets early, and link patients in need of community care to local governments.

Local governments will provide tailored integrated care services such as home visits, home care, and lifestyle support to linked patients to ensure continuous management and support even after discharge.

Jeonbuk Province and the agreement institutions have agreed to strengthen the identification of target individuals and service linkage, and to cooperate to ensure the stable operation of the local community integrated care system.

Bang Sang-yoon, Director of the Welfare and Women's Health Department of Jeonbuk Province, stated, "As Jeonbuk has a rapidly aging population and many medically vulnerable areas, the linkage of hospital care to community care after hospital treatment is crucial," adding, "Through this agreement, we will establish a medical safety net that extends from hospitals to homes and support discharged patients to live stably in the community."
